STUDENT'S STUDYING OUTSIDE THE NWT

Students attending school outside the NWT must notify the
Department of Health and Social Services
EACH ACADEMIC YEAR, IN WRITING
and provide the following information
in order to remain covered under the NWT health care plan:

 

 
1) Name
2) Date of Birth
3) NWT Health Care Number
4) List of dependents residing with you while you are gone
5) Permanent address in the NWT
6) Temporary address outside the NWT
7) Copy of admission letter from institution
8) Start date of semester or the date leaving the NWT
9) Expected date of return to the NWT
10) Contact telephone number(s)
11) Signature

 

  Also, please advise our department if you require another healthcare card as health facilities may require you to present your card when seeking health services.

REMEMBER:  As long as you currently have valid NWT healthcare coverage, are a permanent resident of the NWT and inform our office each academic year that you are out of the NWT as a student, you are eligible for NWT healthcare coverage.  Therefore, DO NOT apply for healthcare coverage in another jurisdiction if you are a student.
